Salary: $95,000/year

Schedule: Monday - Friday, 9:00 AM - 5:00 PM

Position Overview:We are seeking a highly qualified Clinical Team Lead to manage the operations of our Critical Time Intervention (CTI) team in collaboration with New York area hospitals. This program is designed to provide intensive support to high-frequency users of emergency department services, addressing both medical and psychiatric needs. The Team Lead will oversee a multidisciplinary team and serve as a liaison between hospitals and the program, ensuring clients receive comprehensive care during their transition from hospital to home. This is a pivotal role for an experienced clinician who is passionate about leading and building impactful programs.

Program Details:

Program provides intensive, phased services for up to 138 clients, with staggered enrollment to ensure manageable caseloads. The goal is to support clients through discharge planning, home visits, and follow-up care for 6-9 months, addressing issues like medication management, food security, and home safety. This role will require both office-based work and hospital visits, with some flexibility for community-based meetings.

Key Responsibilities:

* Program Management: Ensure program operations align with the model’s fidelity and adhere to OMH and Medicaid standards.
* Team Leadership: Supervise a team of 3 clinicians, 5 case managers, and 1 LPN, providing ongoing staff training and development.
* Hospital Liaison: Act as the primary contact for hospitals, managing client referrals and coordinating care from hospital discharge to home visits.
* Client Care Coordination: Oversee the clinical and case management aspects of care, ensuring timely assessments, follow-ups, and service planning.
* Compliance & Billing: Manage Medicaid billing processes and ensure program compliance with all relevant regulations.
* Quality Assurance: Conduct regular audits, ensuring documentation and service delivery are compliant and effective.
* Crisis Management: Respond to urgent client needs and admissions, coordinating with hospitals and dispatching team members within 24 hours of referral.

Qualifications:

* Licensure Required: Licensed Practitioners of the Healing Arts (LPHA) – LCSW, LMHC, PMHNP, RN, or Psychologist.
* Experience: 3-5 years of post-licensure experience, preferably working with individuals with mental health and co-occurring disorders in a hospital or community care setting.
* Leadership Skills: Previous experience in team management, staff supervision, and program development.
* Technical Skills: Strong familiarity with Medicaid billing, clinical documentation, and regulatory compliance.
* Communication: Excellent written and verbal communication skills.
* Enthusiasm for Innovation: Eagerness to build and lead a growing program focused on improving outcomes for frequent emergency department users.
