Job Summary  
We are seeking a compassionate and dedicated Hospice Nurse to provide high-quality patient care to individuals with terminal illnesses. The ideal candidate will possess a strong nursing background, exceptional communication skills, and a commitment to enhancing the quality of life for patients and their families. As a Hospice Nurse, you will work collaboratively with an interdisciplinary team to deliver comprehensive care that addresses the physical, emotional, and spiritual needs of patients.

Responsibilities

* Conduct thorough patient assessments, including vital signs and medical history evaluations.
* Develop and implement individualized care plans in collaboration with the healthcare team.
* Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
* Provide education and support to patients and their families regarding disease processes, treatment options, and end-of-life care.
* Maintain accurate medical records and documentation of patient care activities.
* Monitor patient conditions regularly, adjusting care plans as necessary based on ongoing assessments.
* Offer emotional support and counseling to patients and families during difficult times.
* Coordinate with other healthcare providers to ensure continuity of care across various settings.
* Participate in interdisciplinary team meetings to discuss patient progress and modify care strategies.

Skills

* Proficient in conducting vital signs assessments and understanding their implications for patient health.
* Experience in pediatrics, geriatrics, or critical care settings is highly desirable.
* Strong diagnostic evaluation skills to identify changes in patient conditions promptly.
* Ability to maintain comprehensive medical records accurately and efficiently.
* Expertise in patient assessment techniques to ensure holistic care delivery.
* Familiarity with primary care practices and protocols relevant to hospice settings.
* Compassionate approach towards providing patient care that respects individual dignity and preferences.
* Knowledge of working within a Level I Trauma Center is an asset.

Job Type: Part-time

Pay: $50.00 - $60.00 per hour

Expected hours: No less than 10 per week
